Service Canada CPP Forms

How to request verification of enrolment for a Service Canada (CPP) - Declaration of Attendance at School or University.

  1. DoԴdzmail or drop off your Service Canada form (ISP-1401) to Camosun as the college does not stamp or sign the form. Service Canada will accept a proof of enrolment document (Camosun Letter of Enrolment) to confirm your attendance.
  2. After your first day of classes, request a Letter of Enrolmentٳdzܲ. Your Letter of Enrolment will be emailed to you within seven (7) business days of payment.
  3. See the mailing instructions on the last page of the Service Canada form (ISP-1401).
    • Important: Do not complete Section C on the Service Canada form.
  4. Submit your Letter of Enrolment along with the Service Canada – CPP forms to Service Canada. Camosun does not mail or email letters or forms on the student’s behalf.
